Assessment Process

Assessment Details

Should the client wish to proceed, a risk assessment and assessment of need will be undertaken in the client’s own home. This will be done by an appropriately qualified member of staff who may be accompanied by a member of staff from Dorset Adult Social Care (where applicable) and family members if required.

A personalised care plan will then be drafted in consultation with the client and family (where appropriate) using the assessment undertaken by Better Care at Home. A copy will be placed in the client’s care folder at the client’s home for reference.
